Fraud, Waste, and Abuse Reporting Form
SBCC is committed to maintaining the highest standards of integrity and accountability. This form provides a confidential way to report suspected fraud, waste, or abuse. All reports are taken seriously and will be reviewed promptly.
You may choose to remain anonymous when submitting a report.
- If you choose not to identify yourself, SBCC will not know who submitted the report. Identifying information is not shared with SBCC. The WeTip system enables secure two-way communication so that SBCC can ask follow-up questions without knowing your identity.
- If you choose to identify yourself, SBCC will know your identity and may contact you for further information. Confidentiality will be maintained to the extent possible under the law.

After you submit your report, you'll receive a confidential Case Number. Please save this number for future reference.
